How to add an item to IE's right-click menu

Anyone knows how to do this? I found some info on MS's webpage, but it didn't seem to work for me. Additionally, I would like to link it to an executable.
MS says to add a key in the registry, however a lot of programs (like download managers) I have added items in IE's right-click menu yet didn't add anything to my registry. I conclude there must be some other way.

Any ideas?

For Explorer I only know the easy way.

C:\Documents and Settings\youruserprofilenamehere\SendTo

Stick a shortcut in there if you are trying to send a file to an executable.

If you are trying to do it in IE's right click menu I am pretty sure the link that ElDonAntonio has provided works fine. Considering I have a quicklaunch bar I only need to send stuff over to a program easily. If I just want to open a program quickly I use that
