How should retaliation in baseball be handled?

I remember reading about hockey fights, that officials like to let them duke it out because if they dont' people might start retaliating with sticks and their ice skates instead. How should retaliation be handled in baseball? I can't recall any serious injuries to hockey players in a fist fight, but I know they get penalties for it at least. A 90+ fastball to any part of your body can put you out of action, especially since a lot of pitches hit wrists and hands and the like.

People start fights in hockey because there isn't any reason not to. Sitting in a box for two minutes? Gimme a break. HUGE fines, game suspensions, and dismissal from the league for repeat offenders. You don't see fights in baseball NEARLY as often as in hockey because the players have good reasons not to.
