I change jobs when my experience level and skill set becomes more than what my employer is willing to pay. I find a new job, get an offer, and if I enjoy the company I am currently at, offer them the opportunity to match it or I walk.
From my experience, you won't ever get a big raise unless you either change positions drastically, or get a new job at a new company. That entire "working your way up" only works if you advance upward in promotions, which isn't really something I want to do. I much prefer doing to actual development and less of the management of others at this point in my career, so it is usually on to the next job.
Really depends on your line of work. I know the need for GOOD developers is always huge, but that is mostly because the majority of them being pumped out as code monkeys are crap these days.
