How does cool 'n quiet work?

As far as I can tell my cool 'n quiet isn't automatically changing my clockspeed to its maximum in cpu-intensive programs, it just stays at 1 ghz instead of 2.2 (according to CPUZ). I've got it set to automatic mode. At what point does cnq actually kick in?

I would suggest using AMD's C'n'Q' dashboard and then running Prime95 to see if it is working properly.

Try running a program that continuously utilizes your CPU then check CPU-Z again.
