It means you have a lack of iron in the blood, which means (I think) less haemoglobin. Its haemoglobin which takes up the oxygen, so if you have a haemoglobin problem, you can;t take up as much oxygen in the blood, so you don't get enough oxygen/your heart has to work harder.
