How do you guys organize your photos?

Separate directories by subject with sub-directories by date if needed. ThumbsPlus allows pics to be grouped into galleries w/o moving the original files but I don't use that very often. In the past I've used the ThumbsPlus database functions to make the images searchable but I'm too lazy to keep it up to date.
