I have noticed that if the info that you have for the file differs too much (and that too much aint much) from what iTunes Stores has it as, it won't download. It did get a lot of mine, but a bunch of my stuff didn't come from CDs, so it doesn't always have enough information for itunes to work.

I use MediaMonkey to put the album artwork into my MP3's. It uses Amazon to find the images. I just check the option to save the artwork inside of the MP3. THen when you load the songs into iTunes, it automatically processes the artwork.
