Social Security has nothing to do with the name change process (other than you having to tell the SSA after your name is changed) since name change petitions are handled by the state judicial courts. You can usually get and file petition forms from the county seat municipal court, then if your case doesn't fall under common categories (e.g. naturalization, marriage, etc) they call you up in front of a judge or appointed clerk to plead your reasoning.
