How do proximity cards work?

You know the cards you put in front of a little sensor and it unlocks the door or whatever. How do these cards work?

Are they all individualy coded?

Lets say I have access to certine area and I can unlock a certine door with my card, and many other people are able to do the same. If I would lose my card, what would happen if they wern't all individually coded. They would need to change the codes on all the cards that people still have in order to stop unauthorized access to that area...

Anyways in general, places that use this type of system, do they usally give indentification to each individual cards or just a batch of then that are identical?
