How Do I Make an Item Request Form that Will Email all My Site Members?

I would like to make a request form on a website that entails:

1) Vistitor filling out some fields for a requested item
2) The request gets emailed to all the members of my site

2) do you have an existing site?
3) if you do and you made it yourself then this should be easy.

I do have an existing site and it has a bunch of PHP.

I just found: PHPforms.net and tried the demo. I think I will go a head and use this one unless you think otherwise or recommend something better.
