how do HTPC remotes work?

Do you have to drill a hole for the remote sensor,or do HTPC cases already have a hole to stick one in.Or do they have a photodiode mounted already in the case that hooks up to your video card(if it is a videocard/remote combo)?Or do you just tape it to the front of the case(that would be really tacky.
 
Mine has a receiver attached to a ~4 ft USB cable.


The actual receiver end fits very neatly under the recessed corner of my tv
