How do crowfoot wrenches work?

I couldn't find any (Google/Youtube) videos of crowfoot wrenches in action.

I understand that you're supposed to attach an extension to the square opening of the crowfoot wrench. Do you then attach a ratchet to the end of the extension? If so, then is it analogous to the setup of the pedal and gear on a bicycle, i.e., the ratchet would rotate the crowfoot wrench similar to the bicycle pedal rotating the bicycle gear?
 
I've used them in sinks. I just attached them to the end of a long extension and was able to swing the socket down below the bottom of the sink.
