Edit: The rebate form says either the top of the box or the title page from the instruction manual. Guess I'll be sending in the title page of the manual.

I was able to peel the UPC sticker/label off of the tin. I placed that sticker on a piece of paper and submitted it.

When I got my MS Flight Sim, it came with a rebate coupon stuck to the front of the box. I pulled it off and liked what it said about proof of purchase, so I used it instead of the printable form. For a proof of purchase, all that they wanted for this form was a copy of the receipt and the little stick-on rebate form.

Has anyone received both the $20 rebate and the $10 upgrader rebate? I already submitted and received the $20 rebate. I won't bother submitting the $10 rebate if MS has been rejecting them.
