HELP: Requesting final value fee on ebay

Hi,
i cant seem to find the form that lets you request the fee after you mutually agred to stop the transaction. The one i found keeps blabbing on that you need to fill in NPB for first which is not the case. help me out

I also like how they discreetly hide the Contact eBay forms

EDIT: I think you have to go through the NPB alert before you can request a fee credit
