Typical format what?

I have always just called and said I want to cancel.

It's actually a secured Visa through my credit union and they said I need a letter with a signature stating I'd like to cancel my Visa.

I have finally come to my senses and realize you and other "credit" companies are the prime cause of the downfall of Western Civilisation as we know it.
Since I am loathe to be part of my own destruction, I must insist that you cancel my Credit Card #---- ---- ---- ----, as soon as you receive this letter. (Which is sent w/ delivery confirmation)
Please know that I do not wish to receive any unsolicited offers from you or your minions from Hell, so remove my name and address from your marketing lists.
To recap, cancel account number xxxx-xxxx-xxxx-xxxx upon receipt of this letter.
Remove my name from your customer / Marketing lists. Do not call me to re-instate or send any unsolicited offers from your company or any third party you sell names to.
Please send a confirming letter showing the cancellation to my return address as soon as this is accomplished.

Of course, signing YOUR name will mean more, but this letter is what I send.

Dear sir or madam,

I'm writing to cancel my VISA card, xxx-xxx-xxx. I'm sorry we have to part ways, but this couldn't have come as a complete shock. We had our good times together, like when we purchase a round for the whole bar. But we also had the bad, like when I sent in my payment 2 days late and you hit me up with those charges. I would over all say things were good, but I've grown and believe it is time to move on. I pretty sure you'll do good in your credit card endevour. Love xxx.
