help identify this card

I found a video card here at work and I have no idea what it is. It looks like its in new condition.

There is very little markings on the card. I've googled all of the markings on the PCB and even the RAM chips but neither returned anything helpful.

there's a sticker with a barcode and V4000/D/64C and then I assume the serial number which is 92040700980.

It's a fanless blue PCB card with a silver heatsink. It has a VGA and S-Video port.

It is has an FCC ID on it you should be able to look up the manufacturer.

This might not help you identify the product by itself, but it would certainly be a start in the right direction.

Pop the heatsink off and check the GPU markings. That should narrow it down quite a bit.
