I got one for ya:

I was having sex with a girl that worked at my job. Her bf that she lived with also worked there. The bf bought her a new car that was manual transmission and since I knew how to drive stick, they both suggested that I teach her how to drive it. Well needless to say every time I was teaching her how to drive it, she was managing a different stick. Well anywho, one day that I was teacher her how to "drive" at my house her bf called and said to pick him up from work so they could both go to his cousins house for a party. She said she didn't want to go cus she was tired from work. He said ok and that he would just get his cousin to pick him up. Well his cousin was a gang member and when they were arriving at the cousins place with the bf in the car, some rival gang members shot up the car killing the bf and not even hitting the gang member cousin. Needless to say I felt a little responsible.

There was a Minnesota Army National Guard soldier that was killed at Ft. McCoy, WI, some years back. He was in my unit, and we were operating the night time rifle qualification range. I had been out on the firing line all night assisting shooters, so I requested with the range NCO to switch with someone in the ammo shack to reload magazines so I could get a break for a while. He sent PFC David out in my place. A soldier from another unit who was there to qualify did not follow instructions, and accidentially shot PFC David in the neck. We shut the range down as soon as someone called for a cease fire, and David was airlifted to the nearest hospital by helicopter. He died shortly there after, despite our most experienced combat lifesaver's best efforts to control the bleeding.

I do feel directly responsible for his death.

You were fucking some guy's girlfriend behind his back while telling him you were teaching her how to drive a manual transmission. Then, when he needs her to drive the manual transmission, she of course can't, so he has to and he ends up getting killed... I don't know whether that makes you the most awesome person ever or the biggest douchebag in history...

Probably not really, but I feel like it. When my father was in the hospital and we were making the decision of life support, I was the last one to be asked of the people whose opinions legally mattered. They need 100% agreement of the empowered people, so I could've refused to pull the plug and kept him going. I know it would've been a bad decision, but the simple fact that I could have makes part of my brain feel like I caused it.

I've been a paramedic for 27 years, so yeah, there's been quite a few die, but none intentionally.

Not to my knowledge but if it happened on the job I wouldn't know anyway. I've never seen it written down anywhere but I'm told that the official policy is that if one of us makes a mistake with serious consequences we'll simply be terminated with no explanation to keep us from killing ourselves out of guilt.
