Doesn't happen often, but it has. I've been rejected by 2 girls, both were pretty blatantly going to happen, but I like to aim high.
 
I got turned down today, some tall gorgeous blonde chick. I had a car rental, she was dropping me off to pick up my car from the body shop.

When I asked her if she was interested in meeting up, she had this anguished look on her face and was like "ohhh . . . i don't know . . . i have a boyfriend . . . i'm really sorry . . . thanks". The way she said it was really cute and hard to take badly.
