You do. If you come here legally, and are not a refugee, you have to pass a medical test. Not only that, your visa application is at the discretion of your interviewer. You have pass the interview, before they give you the visa. They determine if you everything looks right (you claim to be an engineer, but your clothes and attitudes match that of a peasent), does this person have an income potential in the US, is there a criminal background, etc. Getting a US visa is very hard.

Yes, when I immigrated to US, I had to go through this. And you have to do it in the home country before you ever set foot in the U.S. They do pretty much what you listed for Canada, plus they ask if you have mental disease, or have been a Nazi, Communist, or Polygamist at the interview.

There was a family in line before us and I over heard their interview. The guy was in the fishing business, and had worked on an Iranian fishing boat. They probably asked him 10 questions regarding that, making sure he wasn't a spy or had links to anything suspicious. Oh and the interview was in English. Don't speak English? Too bad, no visa for you.

at first i wasnt so sure about this other method myself but i spun my computer chair around.. planted my feet firmly on the carpet in my bedroom.. pushed myself up to a standing position.. and headed outside. the sun was oh so bright i wasnt sure i could withstand the rays on my very white flesh. But after being smothered in SPF75 sunblock i made my way outside! it was so weird. When people laugh noises actually come out.. not LOL or ROFL. I couldnt believe my ears.. oh what a wonderful world there is to discover. :p
