I was very concerned about our inability to satisfy the documentation requirements on the rebate form, so I spoke with Frank Zhou [(800) 698-5798 x216], the rebate program manager at KHypermedia, the manufacturer of the Value Disc CDR's. He was adamant about the rebate *NOT* being honored without the exact documentation specified on the rebate form. OfficeMax does not and cannot provide the required documentation anywhere but in their retail stores. It appears this offer was intended to be B&M only and was mistakenly offered online. The product and rebate offer disappered from OfficeMax's web site this morning. It's now back, so???

After two days of talking to OM representatives, I received a call this morning from Andrea [(216) 921-6900 x16086] at the OfficeMax Corporate HQ in Cleveland confirming the problem. Her instructions to me were to submit the completed rebate form, original UPC code, and packing slip to the following address where it will handled as necessary to ensure the rebate is not rejected:

It's my understanding that the rebate submission will ultimately end up being forwarded to and processed by The Express Group in Walled Lake, MI [(800) 527-3342].

 
I'm waiting for a call back from OfficeMax about this too. Haven't hit any dead ends so far, but we'll see.

Update: I was called back by an OM manager. She spoke to Frank at Kypermedia concerning the rebate for online purchases. Their decision as of 2/27 is to accept the rebate with the following:

Rebate form downloaded and printed online.
Original UPC
Original Packing Slip

Nothing else is needed, since this is all we have. I was told to put a note with the form stating that the rebate is "valid with the enclosed documents based on the conversation of 2/27/02 between Frank Zhou and Lisa, call center manager at OfficeMax." Lisa was probably the most helpful CSR I have ever dealt with. She gave me her direct line and promised that she would make sure I got my money, even if she had to "go down there herself."
