Are they worth it? And is there one that stands out above the rest? Looking at all the different ones and I just don't know if i'll use it since I use the top of the desk at the moment.

Confused please help

Depends. I found that my desk was a better mousing surface than the cloth and rubber things I'd used for so long. But that was probably due to it being an optical mouse. It slid easily over my desk, and had excellent tracking. Then I found the C4 mousing surface, courtesy of someone posting here, asking for beta testers. I came in aftr the beta test, and bought one of the adhesive-backed pieces. Most expensive mousing surface I ever bought (paid $1 each for my previous mouse pads), but it's also the best. I wouldn't call it life changing, but it is quite nice.
 
I used to use the desktop for my mouse but now the area looks like crap (the feet of the mouse rubs off the desk finish) and the top left foot of the mouse was starting to get destroyed. So I whipped out an old mousepad lying around the mouse somewhere nad it works fine.

My desktop is wood though, so if yours is something else (plastic, vinyl, corean, etc) then I don't know about it.
 
I've tried dragging my optical mouse across my laminated particleboard desk and it drives me bonkers. It's very inconsistent. The "gamer" mousepads are all too slick for me. Cloth and rubber for life! 😀
