The "problems" in Iran predated Carter. I guess one could say Carter failed in Iran. Then again which President succeeded in Iran . . . Reagan? Granted, Ollie North probably has an interesting POV on the matter.
I'm a touch uncertain about what Clinton "failed" to do in Iraq. Although the US clearly had a posture of "regime change" under Clinton, most people had enough sense to realize that "regime change at any cost" was just plain stupid.
Curiously Bush41 might have been the sole successful President in Iraq. He went into the region with specific goals, achieved them all, and paid pennies on the dollar.
