Thanks for the deal joker, but for anyone who's paranoid about security, your info can be viewed simply by looking in the source and copying the 'save file' url. It's suppose to be password protected.
 
Woah. Like mrCide said, there's a 325K and counting text file with everyone's name, address, phone number, email address, IP, all that fun stuff just sitting in the clear for anyone to take. Do these people work for Digital Convergence or something?

Update: At least when they tested out the form, someone had a sense of humor. One of the first responses to the survey was "George Michael" "gmichael@therestroom.com"
