Either way having it face the other direction would have made it far less accessible, and my experience tells me this is a legitimate concern. I've already swam in shit down in that basement.

A way to save some space, if you need to keep the 2x6 wall tight to the side of the stairs while still covering the stack, is to expose the flat cap, build the wall flush with it, and paint over it. If done well the 4" exposed cap isn't much more obtrusive than an access panel (especially if it's down near the floor like it should be haha).

All lot of people like the mysteriousness of an access panel though, and it's a good place to stash the weed.
 
Honestly I would just have an access door and call it a day. I personally would have put it lower, and maybe have a small extension and just have it stick out from the wall. The Y ones are better too, easier to get a snake through. But it's done now and don't really blame you if you don't want to change it now.
 
Just read the entire thread. I don't understand what the problem is. If the access panel is large enough, you can get any kind of wrench you like on the cover.

As others have asked ... why is the cleanout that high off the floor? I can't imagine clearing a clog above and having all the sewage and water in the stack falling out of a cleanout four feet off the ground.
