external heatsinks?

heatsinks are always inside the case and then a fan blow air around the heatsink out of the case. if devices used heatpipes and external heatsinks (with a grill/cage around it to avoid direct contact) wouldn't cooling be more efficient and quieter and possibly fanless?

There is one like that for video cards. Not sure about one for CPU's, I've never seen one.
