Errors with VWIC-2MFT-T1 cards?

Every single one of these cards I run across is taking input errors on the serial interface and path code violations on the controller. Clocking has been verified and all my extended tests to the CSU are 100% with multiple stress patterns.

I've moved the cabling over to use the other port and same thing happens. Layer 1 has been verified multiple times, are these cards known as having bad batches?
 
Easiest solution is to use a loopback plug and run your tests that way. Will immediately tell you if it is the card or not.
 
We had a similar issue with one of those cards and a circuit from MCI.

They swore up and down that the circuit was good and that it was our hardware. We ended up swapping everything out. AT&T swapped out the smartjack. We reran the extension. Replaced the router and the WIC. Still had issues.

We knew it was an issue with the circuit and not our hardware, but we had to prove our side before they would listen.
 
I've ran to hard loops at the dmarc, and I've ran H2H to a test set at the end of the cable. No errors.

As soon as everything is normalled up, the interface is still incrementing slow errors.

I don't think it's the circuit, but I'm gonna place an internet analyzer between the smartjack and the CSU and leave it there for a day, see which side the errors are coming from, if any.
