Is [one of the forms] singular, so we use "is"?
Or, is [forms] plural, so we use "are"?

If I am correct, we should use "is"... but it just sounds stupid with a plural word right in front of it.
 
take away "of the forms" that will answer your question. because the sentence still makes sense without it.

Bingo. If people would just delete the non-essential stuff, 99% of grammar questions would be answered.
