Do you know where [one of the forms] is?
or
Do you know where one of the [forms] are?

Is [one of the forms] singular, so we use "is"?
Or, is [forms] plural, so we use "are"?

If I am correct, we should use "is"... but it just sounds stupid with a plural word right in front of it.

As always, disregard non-essential stuff, like prepositional phrases. "Do you know where one is?"

take away "of the forms" that will answer your question. because the sentence still makes sense without it.

Do you know where one is?

Bingo. If people would just delete the non-essential stuff, 99% of grammar questions would be answered.
