Dying memory

Not too long ago, I was playing GTA3 when all of a sudden, my system froze up.. so I rebooted and then I started to get BSOD after BSOD. To make things short, I figured out that one of my PC2100 DDR modules from Crucial was causing the BSOD's. So I took it out and now the BSOD's are gone..

The problem is.. this is the 3rd stick of memory that has died on me since November. They have all been 256mb PC2100 modules from Crucial. On my Epox 8KHA+, I have had 2 modules die in the middle slot and now this one dying in the first slot from the left.

I was wondering what my problem might be.. could it be my mobo.. or is it my power supply?

System Config:
AMD Athlon XP 1700+ ( not overclocked)
Epox 8KHA+ (4.38 VIA 4in1's)
256mb PC2100 DDR Crucial (on Turbo setting in BIOS)
Gainward GF3 Ti 200 (30.42 Detonator)

These are my voltages:
Core 0 - 1.79v
Core 1 - 0.05v
+3.3 - 1.50v
+5.00 - 4.78v
+12.00 - 11.86v
-12.00 - 1.87v
-5.00 - 0.28v

I know that my 3.3 is off.. I just don't know why and if it's really serious or not.
