Dragon Age - why are some of my weapons disappearing?

This has happened to me three times now - I'll be engaged in battle and suddenly notice one of my party members is fighting with their fists. The weapon they were using is gone, disappeared, never to be found again. Twice this has happened with really fucking nice weapons too (like that two handed sword that kicks the shit out of dragons). Anyone know what's going on here?

Are you sure they aren't somehow changing weapon slots? I had that happen a few times.

You sure they aren't switching to their secondary weapon slots, or primary if you have them on secondary?

If that isn't it, then that it weird. Did you install mods, or did you get the weapons from the DLC?

I've never seen that happen in multiple playthroughs. Accidentally switching slots seems the most likely. After this occurs, when you open the characters inventory, is the weapon still equipped? Is it moved to the party inventory?

Have you checked the effects on the character immediately after it happens, in case something has dropped your stats below the minimums required to equip it?

If the weapons/items disappearing happen to come from downloadable content, or even from modifications, then either make sure that the mod(s) is installed (or re-install it) and/or that the downloaded content is properly activated and that you are logged on to your account in the main menu. If the items are from the vanilla game I really don't see what could cause that problem, perhaps other than some strange compatibility issue between the game and one or more mods fiddling with the items of the game that you happen to see disappear from your inventory. The last possibility I can think of is an accidental switch from the weapons slots during game-play. I for one never experienced that issue at all, and I use official DLCs along with custom mods.
