I am unable to go to the system menu and have any changes take effect. I try to set 1280 X 1024 with 16x AA. I restart the game as suggested at which point in time, the resolution falls to 640 X 480. Any suggestions on why this could be happening?

The config is the one in the signature.

I have the exact same issue with an Opty 165 / 1900XTX / DFI Ultra D setup. What works for me at least some of the time... is only change one thing at a time. Go in, set your resolution ONLY; back out; restart game. Go in again, Add 4xAA, back out, restart game. Go in again, increase AA again, wash, rinse repeat. Sometimes that makes the changes stick.

But when that started not working, I'd set the resolution in the in-game menu, then use Catalyst CC to force the AA. Since you have Nvidia card, I would think you could do the same thing with whatever Nvidia's equivalent to CCC is.

It is quite disappointing that I have to take this route. I updated the game with the latest patch but that didn't help either. The control panel for the GPU driver has too many graphics settings for the game and I am not sure if I want to tweak all of those options. Would have been nicer to change everything inside the game and be done with it.

I totally agree. It pisses me off that id Software never fixed this, and I recall people complaining since initial release. They HAD to have known about it. But, since I like the game THAT much, I grudgingly accept that transgression. Still... wholly unnecessary. The thing is though... once you DO get it set right... at least through the in-game menus... it seems to stay indefinitely until you decide to tweak something. Then ya gotta start all over again.

For what its worth i also have the same issue, though not as bad, with Quake 4... id needs to fire their menu guy.

Your 8800 GTS cannot handle the stress of such a high resolution. :)

I never experienced this issue.

You're kidding, right? My 6800GS played D3 at that res with good frame rates. I don't think I applied AA though. If anything, he's applying too much AA.
