Most explosions do NOT make fire. They might set off a fire if there were flammable things in the vicinity and other objects were propelled into them in the correct manner.
Hand grenades dont make flame, they make fast moving shrapnel. Mortars are the same. Most bombs dont flame, homemade or military. Powder exploding generally does not.
Interestingly, Die Hard 3 actually had a very accurate explosion right at the very beginning.

Doesnt matter how powerful the drug is, that shit has to cycle thru the body first. Even if he got the artery leading right to the brain, its still going need a short time to take effect.
One of my annoyances that most people seem to ignore is the way pills are portrayed in movies. For example, if a character has a headache, he'll swallow a pill and *instantly* feel better. I have yet to see a pill that kicks in literally instantly!

Stuff like this is one of the more subtle ones, but I see tons of people expecting their Tylenol to kick in faster than it ever really does.
People are totally inured to violence. I guarantee at least half of the people in real life would simply freeze when shot at, near explosions, eye witnessing assassination, splashed with blood and gore or, just slapped around.

In movies (and television), you'll have characters begin dialogue serving as exposition in one location, have a revelation that causes them to leave that location, jump cut to the characters traveling or at another location and they continue their conversation as though no time has elapsed. I mean, sure, I guess in theory you could do that, but my wife gets really pissed every time I tell her "hold that thought for 45 minutes so we can resume without missing a beat for dramatic purposes." Turns out the 45 minutes they cut is just one character repeatedly hitting the other and calling them a jerk.
and most of the stuff happening in Captain America 2 in particular.
Next up, we hear about how unrealistic it was for Bruce Willis to find out he was dead at the end of a movie about a boy who could communicate with spirits!
