One of the problems I've long had is perfectionism. A general feeling that if it can't be done to A+++ quality it's not worth doing, even if you actually have to do it.

Not exactly. A true perfectionist will spend inordinate amounts of time trying to get it right, while I on the other hand never start ;^)

Does that include never starting on things you REALLY need to finish like critical work related stuff, because you feel you can't perfect them? Or are we just talking personal stuff and recreational things?

More recreational things. Work has to get done, and usually quickly. I was a surveyor, so everything you do has error. There's no such thing as a perfect measurement. The trick is to put in the right amount of accuracy for the given job.

That option isn't there because that's not what perfectionism is. Perfectionism is something that hampers people. Usually it prevents people from getting things done at all. The concept of doing something "to the best of my ability" doesn't exist for a perfectionist because there is always "to the best of my ability" + 1.
 
When you actually do things are they perfect? otherwise, get over yourself and accept that you're not very good at doing things perfectly and move on.

wat? Perfectionism is a mental state and has nothing to do with the actual quality of the work.

You're assuming no one is perfect. Perfectionism hampers lesser people, to me it's motivation.
