Fee free, & you can create/cancel them at will.
As for MBNA it seems that BofA will be inheriting MBNA's system, not the other way around. So perhaps they will keep theirs.

Staples B&M carries Amex gift cards. As long as the online merchant accepts Amex then you're golden.
