Staples B&M carries Amex gift cards. As long as the online merchant accepts Amex then you're golden.

I dont know as I've only seen them but never purchased one.

I have used WebCerts though and those you can add whatever name/address you want.

AMEX used to have the service, but not anymore. Aside from Citi, Disco offers virtual numbers too.
