One of them gives you an email address ending in hotmail.com, the other ends with msn.com. :) Hotmail also would put ads into your emails with the free service (usually an ad for signing up with MSN), whereas since MSN is an ISP and you're paying for the service, there are no ads inserted. Hotmail is also web-based, while MSN mail would be done through the MSN Explorer or Outlook Express.

My Father has just opened an email account with MSN but does not subscribe to them instead to a local isp.
He accesses it through Outlook Express but if what you say is true then does that mean he can not access his email from a web browser other than MSN Explorer?

Most likely he will not have access to his MSN mail account except through Outlook Express. Microsoft uses a special type of authentication (Secure Password Authentication) that nobody else supports. Hotmail accounts are not the same as MSN accounts. Hotmail is accessed through the website. For a short period, Microsoft had Hotmail accessible through Outlook Express, but they don't allow that anymore.
