Dial up modem config problems

Here's the situation. Computer full of crap. Worked fine, but slow. Reinstalled OS (Windows 98 2nd). Reinstalled drivers. Now the first time I booted up I was able to connect to the internet. But after a reboot I get an error saying the "modem is not configured properly."

Everything looks fine when I poke around. Modem is on Com 3 and is V.90 k56Flex HSP PCI Modem by PCtel. The only way I can get it to work is to reinstall driver and reboot! Yet anytime I reboot again the modem will give problems. There is no warning sign in Device Manager and it says the driver for the modem is fine, so I'm not quite sure what is going on.
