Question DHCP issue in Switch

I have two DHCP Switch in my network with two different Vlan i'e VLAN11 and VLAN9.DHCP will be provided IP to the end user by VLAN they have configured. But the problem is some clients on the VLAN11 switch will get the IP of VLAN9. I don't know how it has happened.
 
What network gear do you have and what's the port config for the ports giving out wrong addresses

We have cisco L2 Switchs where i configured the DHCP I have configured the access port the switch .In the VLAN 11 Switch it get ip from VLAN9 DHCP Switch IP.
 
interface GigabitEthernet1/0/5
switchport access vlan 11
switchport mode access

Looks ok, so gotta look broader now. Here's a thread I found that may help.
