I purchased a Creative Labs Audigy 2 Zs Internal Soundcard from Amazon.com a few weeks back and there was a $20 rebate form on their website.

I forgot to print it out and now it's no longer on there. Does anyone know if there's an Amazon rebate form data base? I really don't want to lose $20 here.

It might still be in your history. Go look at another rebate form, erase part of the URL and let it auto-complete, see if you can find it that way.
