Credit Card Gift Card Questions

I am not sure if "gift card" is the proper term or not. I am referring to the disposable credit cards that I saw advertised on television at a friend's house.

Where do you get them? B&M stores, online, or what?

Are there per purchase fees associated with them or just the initial purchase fee?

Do you have to tie a name to them, or could I pass it off to my nephew when there was a certain amount left on it?

Can they be used online? In reference to the name question, what do you do when orderig from a website with one, assuming you can?
 
Both, and Banks, and Credit Card Companies (Amex lets me order them fee free).

Prepaid debit/credit card can be purchased most anywhere, drug stores, super markets, etc. Usually the fee is around $5-10 per $100.
