Cut the one out from the box and send that in (make a photocopy of everything in case things get lost).

You can save the one on shrink wrap just in case, but I'm pretty certain it is going to be the one on actual box rebate processor expects.

I sent in the one from the box and they accepted it. I don't remember mine even being shrinkwrapped.
