concerned that I may not be extended a full time offer at the end of this internship

I just had a mid summer evaluation and it wasn't too bad. My overall performance rating is "Meets Expectations" with some strengths and weaknesses
My immediate supervisor (who decides my fate) went away for a vacation during this month and he works from home 2 days of the week so I don't have much interaction with him. I mostly work with little supervision...
He told me that the database that I'm currently creating will be used to evaluate me at the end of summer. I've never used MS Access in my life before I got here. I'm learning as I go
I don't know if he even likes me.
What type of intern should I be to secure a full time offer?
it would be nice to be extended an offer months and months before graduation even though I'll have few other options available
any advice?

I think while performance is important, they know you're an intern and have a lot to learn. Try to show you're willing to learn, and take initiative where you can. If you come up with something useful that wasn't expected it will definitely leave a good impression. Basically, unless you screw up horribly, it will be worth hiring you over another guy (with equal qualifications) since you will be familiar with the team and projects.

"Meets expectations" may not be bad. Depends on the company. Some have the philosophy that if the majority are rated highly, the ratings are inflated.

What kind of job is this? If you have any kind of technical background, an entire summer should be more than enough to become proficient in Access.

it took me 3 summer internships at the same place to get the database stuff down, but just keep asking for help/work, it shows initative.

my place of employment never hires previous interns as full time employees... so maybe check up on your employers habits in this area... also have you discussed the matter with both your supervisor and/or someone higher in the agency?

Yeah. For my company is varies upon the department. Every department has its own budget and a head count. To extend the head count, we would need to ask for approval from the main HQ over seas, finance, and local upper management of the department to sign off on.

When we merged or got took over by another company, they were more tolerant in extending the head count.

A lot of interns get cut loose where I work. Fortune 500 company known world wide. I'm sure you likely used one of our products in your life time. (I don't mean to patronize you). Though as you can see by now I'm trying to keep my company's identity concealed.

The good thing about being an intern with a well known company in the industry, is that when they get cut loose as expected, the other companies will gobble you up because that's what competition does. They see my company's name on their resume and something will ignite in their eyes.

Our competition loves to offer our current more experienced employees a "little" more incentive for making the switch. Interns are a little different of course because they know you're just another fish in the water, but believe me they like hiring young guns into a company where they can give them the base salary when they're replacing someone else who use to make more than twice as much as the new hire.

Of course the industry I am in is highly competitive and does not include everybody.

So why would anyone intern there?

where else can high school seniors & college students go to make $12-13 per hour starting in a 7-3 standard shift for 3-4 years of work (summer only & year round terms available) and get credits & work experience for doing it?
