Clutch Question

So, I have a question is it possible for a clutch that is engaged to lose grip?

For example, if I am in second gear and press the gass, the revs go up right away but it takes time before it accelerates. Or in a higher gear, the revs jump up, and then come back down.

Sounds like your clutch is slipping and is on it's way out. You must have seriously abused that thing for it to die after 40,000km.
 
Yep, sounds like your pressure plate is on its way out, or the disc is toast.

It is possible though that's it's just way out of adjustment, not sure if that's applicable to your car, but you should go have it checked. If it's slipping, it's going to be increasing the wear, so best to have it looked at ASAP.

Well I was reading some VW forums, and alot of people suggest that there are some issues with the auto adjust cables? Does that sound right?

Could be. Don't delay the investigation/repair. Slipping like that is really not good for it. Fix it now while it's a simple fix instead of major one later.
 
If it's slipping, don't keep driving it, you'll only make it worse.

Try readjusting it first (there should be a half-inch or so of "pedal throw" before the clutch actually starts to disengage), but if it was abused by the previous owner or driven for a long period of time while slipping, you may be in for a new clutch.
