I'd just stick with "Tom's First United Methodist Church"

Churches don't belong to individual people.

Also, it's a United Methodist naming convention. The initial United Methodist church in an area is called First United Methodist Church with the name of the town being placed before that. For example, the United Methodist Church in my area is the Perrysburg First United Methodist church.
