I used to pay $3.95 to insure my Verizon Wireless Motorola v120c. Insurance is handled by a third party meaning it's more complex to expedite the process and worst of all, Verizon have a $50 deductible. I just dropped my insurance because I know that I can get a used phone for $50ish on eBay from people who dropped from insurance or kept their old phone after a phone upgrade.

It's probably worthwhile if the declared retail value of your phone is $400-500(the price you'd pay without provider subsidy).

This is exactly how it works. I bought insurance broke and my phone broke about 2 months later. I had a Nokia 8620( or something) and because AT&T didnt carry that phone anymore the 3rd party place gave me a 6360 that was comparable. Not the same model. I just bought the Nokia 7210 GSM and you cannot get insurance on that phone.
