Catalyst Application Profiles uninstalling

How do you uninstall the CAP? There doesn't seem to be an uninstall option in the control panel-uninstall list. I was quite sure there used to be an option in the older versions.

Another issue I got is a a blank screen after uninstalling the main driver (13.4). Did it happen because I manually disabled ccc from starting up using msconfig or autoruns?

You can always forcibly remove everything with ATIman Uninstaller. Might not be a good idea on systems with AMD cpus, however

Go into the Add/Remove Programs section of Windows. The CAP should have it's own entry there, separate from the main AMD driver. Right-click and uninstall. Pretty simple.

Sorry, I suck at reading. My only suggestion then would be to do a complete uninstall of all AMD software, and then run Driver Fusion to clean any residual files. Then re-install your AMD drivers with a fresh slate. Sorry I can't be of more help.
