I had a bad experience the first time I ordered from them (ordered a video card that said in stock and after over a month I cancelled the order, they took a while to credit the CC, then a month later I got the video card, so in the long run I really shouldn't complain but warning others seems responsible). The last time was ok, I guess (got a video card very quickly that I ordered, but it came damaged, looked like it was damaged during packaging, so it probably wasn't the resellers fault, they sent me a new one quickly as soon as I shipped the card back, but I still had to pay for shipping back). So be warned. www.e-imix.com They are also known as MixPC.com

This is the first 64MB MX card I've seen and at $139 and free UPS 3 day shipping it's a good price, especially from Gainward CardExpert (who makes some of the fastest video cards around).

I couldn't find anything at Gainward.com on this card either, strange that the link for the Hollywood/MX card is dead also, and this card isn't available anywhere.

Seems Inno3d now has a 64MB MX card also, they have one with TV out at OCIE.com for $139. (I think they mislabeled it on their main page as TwinView, but not on the item page).
At Amamax.com, they have a twin view version for $181 (not a hot deal, almost $50 for 32MB).
