QD means "every day," and that last part isn't PM, although it does look like it. It's prn, which means "as needed." They're Latin abbreviations, don't ask me what they stand for though. The "po" means "by mouth" and the funny dotted T's just mean 1 - 3. The first word is "Sig:", which just means whatever follows are the instructions for use.
 
Take 1-3 tabs orally every day as needed

Shitty way to write a prescription, it doesnt tell you as needed for what, or what dosing interval to use during the day.

That's actually pretty standard. Those are the things the doctor should tell you when he writes it.
