Can someone provide a sample service agreement for IT services

Hello,

Would anyone be willing to share their legal service agreements for IT work? I am submitting a proposal to a business and if I get it I will(should) provide a legal agreement for them to sign.

My fees are not hour but flat fee.

I did do a google search but why not ask AT?
 
I've been fixing computers on the side for years while my fulltime job has been in banking/finance areas. Due to some changes coming up(will be moving but don't know where) and because I've always wanted to do IT I have a window of opportunity at my current company. It would be a great stepping stone for me.
