Bad USB ports?

I have a 1TB WD external drive, a 4 GB SanDisk Cruzer, a USB based mouse, and I tether my phone with a USB cable. About a week ago I started getting write delay errors when copying files to the harddrive so naturally I thought my harddrive was going bad. Then I tried copying some files from another computer but it worked fine. I have no problems reading files from the harddrive but I cannot copy files to it. I have the same problem with my flash drive. Sometimes when I plug them in, I see the drive in My Computer but the name of the harddrive has been changed to Local Disk. When I open the drive it shows it as empty. Sometimes when I plug it in it will work fine, other times it won't show up at all. I've had problems when tethering with my phone as well but my USB mouse doesn't seem to have any problems.
Is this a problem with the USB controller and could it be fixed with a USB PCI card?
 
We could use a bit more info, mobo, case, cpu etc. USB ports are they USB, 2,3? You just may need a BIOS it could be anything so a little more info about your computer would give us a little more to go on.
 
Do these issues come up with all 8 USB (looks like you have 2 front panel & 6 back panel)? If it is just the front panel USB ports assuming thats how you connect the flash drive and ext. HDD purely out of convienience you may want to first open up the case, be sure the connecting cable from the front panel USB's to the MOBO is firmly seated.
If all seems well inside the case, open up device manager and scroll down to Universal Serial Bus controllers. A yellow or red icon appearing beside the USB Universal Host Controller or USB Root Hub item will tell you whether you have a problem with the USB controller itself.
If so, before you decide to put money into it, you should delete each USB device from device manager, exit it, and restart your system. Windows will reinstall the USB subsystem and allot of times the reinstallation will fix the problem.
 
I tried all of the above an none worked. I borrowed a USB PCI card from a friend and they're working great so that must have been the problem
